1. If the total value of the stolen property to be reported is over $1500.00, please report it to your local police service.

2. If you have any suspect(s), please make a report to your local police service.

3. Please indicate in the "Property Details", whether you require a police case number for insurance purpose.

4. Please include serial / model numbers for missing items in the "Occurrence Details" section. If the missing item has no such numbers, please note that in the "Details of Incident" section.

5. Please include any related information gathered from neighbourhood inquiries, in the "Witness Statement" section of the report.
